The Anatomy of a Medical Archive

The health archive consists of five parts:

1. Structural process

2. Composition of save files

3. Investigative search

4. Delivery

5. Archive analysis (analytical search)

Structural process

This is followed by the extraction and combing of patient records and associated images, notes, and other data. A selected sample of records is used for testing purposes. A small number of records reduces the time for staff to verify the process. After verification, all patient records are loaded into the archive database. No modifications are made to the data and no data is reformatted. It is stored in the archive database "as is". Data integrity is maintained as it exists in the source database. So the archive is not "converted data". In cases where original data content is required, a true medical archive is essential.

The final step in this structured process is to deliver the Archive database and Query programs for testing and validation.

Compiling medical archive files

The archive contains all data associated with each patient. Maintains relationships between data elements. It does not must or maintain data dependencies. put, the Archive contains only the data that is provided in the source system.

Investigative search

Before any serious analysis can begin, it is necessary to determine what data is available. A survey of archived records will reveal their nature and relative content. Statistics are part of this investigation and may be key to determining further analysis. If the goal of the analysis is to identify trends, it may be useful to sample the data before performing the analysis. Omit, when a complete set of records is required, a complete dataset is available.

delivery

Investigative searches can also be used to select records to deliver to another party. Delivery may be electronic and/or hard copies. Patient records are required for various purposes: medical referrals, patient requests, and subpoenas. They can also be imported into the EMR system.

Archival analysis

To begin an analysis, the archive first provides the means to reduce the pool of records. One example is using search criteria that eliminate records that don't meet those criteria. The search tools are simple. They are designed to provide results that include a refined set of records. Each smaller, more relevant collection of records can be filtered. The goal is to deliver focused sets of records for further analysis. Analysis can be performed using more complex analysis tools. There are many available to choose from. They are available. Each is designed with specific purposes in mind. Some will come with more features that provide more relevant data selection and categorization. Selected archive records are raw data formatted for further evaluation.

summary

The purpose of the Health Archive is to store and present data in its original form while maintaining its accuracy and completeness.

Smart Disc Problems

Problems with the vertebral disc are very painful. They haunt you with excruciating and shooting pain. A vertebral is a plate or disk of cartilage and fiber that contains the nucleus pulpous, a mass of white elastic fiber at its center located between the vertebrae or bones of the spine. Disc problems tend to get worse with age. Aggravation or any injury or any stress can cause a painful disorder known as a slipped disc, herniated disc, pinched nerve, or ruptured disc. In a herniated disc, when the disc ruptures or slips, the white elastic fibers protrude through the cartilage and put pressure on the adjacent nerve root, generating excruciating pain in the affected area. A herniated disc can sometimes be completely painless, but a degenerative disc can cause intense pain. Degenerative means that the condition is likely to get worse over time. Most patients with a degenerative disc will periodically experience continuous tolerable pain followed by increasing intensity of pain that will ...
